The Care Manager will lead assessments, implement tailored care strategies, and maintain thorough documentation, enabling consistent, compliant service delivery across multiple offices.

Qualifications

• 2+ years of experience in homecare preferred.
• High school diploma required; Bachelor’s degree preferred or equivalent work experience.
• Excellent written and verbal communication skills.

Responsibilities

• Develop and implement individualized care plans tailored to the unique needs of each client.
