Job Description: Care Manager/Coordinator (RN/SW)

Position Overview: The Care Manager/Coordinator (RN/SW) assists members in achieving their optimal health through self-management. Responsibilities include engaging pediatric members, caregivers, and providers to assess, plan, and coordinate care, ensuring quality and cost-effective resource use. The role functions as a single point of contact and advocate for members within the care coordination program, overseeing activities within assigned practices to ensure high-quality services in accordance with standards and requirements.

Responsibilities:

1. Assess members through face-to-face encounters and telephone contacts to determine care needs.
2. Complete comprehensive, person-centered assessments including health history, social determinants, and supportive needs.
3. Coordinate physical, behavioral health, and social services.
4. Provide medication management, reconciliation, and adherence support.
5. Identify barriers to care and implement appropriate interventions.
6. Create and share care plans with members and families.
7. Set goals, prioritize, and implement interventions based on clinical progress.
8. Schedule follow-ups, make referrals, and document progress.
9. Coordinate care with community partners, providers, and members.
10. Provide transitional care management and participate in team meetings.

Education & Experience:

• LMSW or LCSW: Master’s in social work with an active, unrestricted license in DE.
• RN: Bachelor’s degree with an active, unrestricted DE RN license.
• 3-5 years' pediatric case management experience required.
• Experience within managed care organizations preferred.
• Strong organizational, time management, and cultural competency skills.
• Proficiency in MS Office and electronic medical records.
• Valid driver’s license required.
