Job Title

Care Manager I, Adult (Murfreesboro, TN)

Schedule & Pay

Mon‑Fri, 8:00 a‑4:30 p; Base: $41,000 per year with additional incentive for language skills and behavioral health experience; $1,500 sign‑on bonus; Mileage reimbursement available.

Job Summary

Provide comprehensive care management services to adults with severe or persistent mental illness (SPMI) and youth with severe emotional disorders (SED). Develop relationships with consumers, address challenges to help them live healthy, successful lives in the community, and collaborate with interdisciplinary teams and community partners.

Responsibilities

• Engage consumers monthly to assess needs, develop and address care‑plan goals, and coordinate care.
• Conduct visits in community settings (home, school, court, etc.) to evaluate needs and connect with individuals where they are.
• Address mental health, physical health, social determinants of health, gaps in care and ED/IP utilization.
• Provide side‑by‑side approach to evaluate and address barriers to engagement in treatment.
• Assist with referrals, linkage to community resources, and advocate for consumers as needed.
• Collaborate with consumers’ support systems and identify/increase supports.
• Follow training handouts and policies to ensure quality consumer care.
• Maintain timely and comprehensive documentation per agency policies and procedures.
• Meet productivity standards as indicated in policies and procedures.
• Actively participate in daily team meetings, coming prepared to share insights, offer solutions, and develop action plans.
• Follow up on plans developed in meetings.
• Participate in monthly supervision, discussing caseload, professional development, and receiving feedback.
• Develop and maintain collaborative relationships within departments and with community partners.

Requirements

• Bachelor’s degree in counseling, psychology, social work, or another behavioral health field.
• For bachelor’s degree in criminal justice: at least 15 college‑level semester hours of behavioral health coursework.
• Valid Tennessee driver’s license.
• Acceptable Motor Vehicle Report (MVR).
• Acceptable criminal background investigation.
• Personal automobile insurance.
