MHC Mental Health Cooperative, Inc. is seeking a Behavioral Health Care Manager to provide comprehensive care management services for children and adults facing severe emotional disorders and persistent mental illness. This role requires engaging with consumers, conducting community visits, and collaborating with interdisciplinary teams to ensure consumers achieve healthy, successful lives.

Job Title: Behavioral Health Care Manager

Schedule: Mon-Fri, 8:00 a.m.–4:30 p.m.

Salary: $41,000 (base pay) with additional incentive for language skills and behavioral health experience. Mileage reimbursement available.

Job Summary

Provide comprehensive care management services to children and youth with Severe Emotional Disorders and to adults with Severe Persistent Mental Illness. Develop and maintain relationships with consumers to help them overcome challenges and achieve healthy, successful lives in the community. Collaborate with interdisciplinary teams and community partners to create innovative solutions that support consumer goals.

Responsibilities

• Engage with consumers monthly to assess needs, develop care plan goals, and coordinate care.
• Conduct community visits (home, school, court, etc.) to evaluate needs and connect with individuals where they are.
• Address mental health, physical health, social determinants of health, care gaps, and emergency department/inpatient utilization.
• Provide a side-by-side approach to evaluate and remove barriers to treatment engagement.
• Assist with referrals, linkages, and advocacy for community resources.
• Collaborate with consumers’ support systems to increase needed supports.
• Follow training handouts and policies to deliver high‑quality consumer care.
• Maintain timely, comprehensive documentation in accordance with agency policies.
• Meet productivity standards as outlined in policies and procedures.
• Collaborate interdepartmentally; actively participate in daily team meetings, coming prepared to share insights, propose solutions, and develop action plans.
• Follow up on plans developed in meetings.
• Participate in monthly supervision, discussing caseload, engaging in professional development, and receiving feedback.
• Develop and maintain collaborative relationships across departments and with community partners.
• Demonstrate the Integrated Care Core Values of connection, innovation, compassion, and integrity.

Requirements

• Bachelor’s degree in a health-related field: counseling, psychology, social work, or other behavioral health discipline.
• If the bachelor’s degree is in criminal justice, the candidate must have at least fifteen college-level semester hours of behavioral health coursework.
• Valid Tennessee Driver’s License.
• Acceptable Motor Vehicle Report (MVR).
• Acceptable Criminal Background Investigation.
• Personal automobile insurance.
