Qualifications

• Bachelor’s Degree in a health-related field of counseling, psychology, social work, or another behavioral health field.
• Valid Tennessee Driver’s License.
• Acceptable Criminal Background Investigation.

Responsibilities

• Engage with consumers monthly to assess needs and coordinate care.
• Conduct community visits to evaluate needs.
• Assist with referrals and community resource linkages.
