Qualifications

• Bachelor's degree in counseling, psychology, social work, or related field required.
• Valid Tennessee Driver’s License and acceptable MVR needed.
• Must pass a Criminal Background Investigation.

Responsibilities

• Engage with consumers monthly to assess needs and coordinate care.
• Complete community visits to evaluate needs.
• Assist with referrals and advocate for consumers.
