Tri County Care Management Organization Inc. in Branchburg, NJ is seeking a full-time Care Manager to support youth and families facing behavioral and mental health challenges.

The role focuses on wraparound services, crisis planning, and coordinating resources to help children thrive at home, at school, and in the community. The position requires a bachelor’s degree in social work or psychology and at least one year of relevant experience; bilingual skills are desirable.

Responsabilidades

• Prepare and carry out short-term crisis plans and ISP.
• Source resources for youth to access formal and informal services.
• Act as liaison between families and providers.
• Travel to meetings at homes, schools, courts, and other services.
• Advocate for youth and families in meetings.
• Follow established timelines for meetings, calls, documentation, and plan submission.
• Maintain accurate electronic health records and youth files.
• Participate in required training opportunities.

Are you passionate about mental health and helping children and families succeed?Tri County Care Management Organization (TCCMO) is a non-profit organization that provides care management (case management) services for children with complex challenges in Hunterdon, Somerset and Warren Counties.We are currently seeking a Care Manager based in our Branchburg, NJ office to provide support to youth who are experiencing behavioral, emotional, social, developmental, and/or mental health challenges.
