Responsibilities

• Act as primary backup for Care Managers during absences.
• Travel to meetings at homes, schools, courts and attend services with youth and families.
• Connect families to therapeutic and community services.
• Complete progress notes, service plans and other required documentation in electronic records.

Are you passionate about mental health and helping children and families succeed?Tri County Care Management Organization (TCCMO) is a non-profit organization that provides care management (case management) services for children with complex challenges in Hunterdon, Somerset and Warren Counties.We are currently seeking a Care Manager Specialist based in our Branchburg, NJ office to support care management activities while learning and practicing the core values of the wraparound process.
