Responsibilities

• Serve as a primary point of contact for clients, families, caregivers, and referral partners.
• Meet with potential clients and their families.
• Perform intake visit with new clients.
• Create care plans for HCBS and IHSS Medicaid clients.
• Communicate with case managers regarding client care plans.
• Understand Medicaid eligibility and PARs.
• Schedule and complete quarterly supervisory visits with clients.
• Assist with the incident/complaint reporting process and ensure timely communication.
• Maintain accurate client records.

About the Role

We are seeking a detail-oriented, compassionate Care Coordinator who is fluent in both English and Spanish to support our clients, caregivers, and Case Managers/Referral Sources. As a Homecare Care Manager , you will serve as a primary point of contact for clients, families, caregivers, and referral partners. This critical role ensures that services are delivered smoothly, communication is clear, and families feel supported throughout their care journey.

Key Responsibilities

• Serve as a primary point of contact for clients, families, caregivers, and referral partners
• Meet with potential clients and their families
• Perform intake visit with New Clients
• Create Care Plans for HCBS and IHSS Medicaid Clients
• Communicate with Case Managers regarding Client Care Plans
• Understand Medicaid Eligibility and PARs
• Schedule and Complete Quarterly Supervisory Visits with Clients
