Job summary

Esperanza Health Centers is seeking a Full-Time Care Manager hybrid role in Chicago. You will connect patient needs with the Care Management Program, support clinical operations and patient care, and coordinate screening for high‑risk patients.

Role involves managing a 62-patient caseload, using EHR systems, and conducting risk assessments. Desired in Spanish and English, with on-site hybrid scheduling and growth opportunities.

Qualifications

• Master’s level behavioral health degree and Illinois licensure.
• 1–3 years of disease management or case management experience.
• Experience with medically and behaviorally complex patients preferred.
• Ability to speak English and Spanish.

Responsibilities

• Identify, track, and manage care for high-risk patients using EHR and health systems.
• Conduct initial care-coordination screening and risk assessment by phone or in person.
• Manage a caseload of 62 patients as per contract requirements.
• Coordinate chronic disease management orders within scope of practice.
• Assess psychological needs and refer to community resources when appropriate.
• Maintain HIPAA-compliant handling of protected health information.
• Develop integrated care plans for highest-risk patients, in-person or by phone.
• Support other care managers and primary care teams for high-risk patients.
• Ongoing clinical assessment, monitoring, and follow-up for high‑risk patients.
• Promote clear communication within care team about patient plans.
• Review medications and coordinate with licensed professional for adjustments.
• Participate in program outreach activities and health fairs.
