VNS Health is seeking a Health Home Care Manager in New York to provide long-term care coordination for clients with complex mental health and medical needs. You will assess needs, develop person-centered care plans, and coordinate with healthcare and community providers.

Responsibilities include maintaining detailed case records, conducting CANS assessments, and supporting families while navigating eligibility and entitlements.

Qualifications

• Bachelor's degree in a human services or related field required.
• Master's degree enrollment preferred for advancement.
• Strong communication and interpersonal skills.
• Knowledge of health care systems and community resources is a plus.

Responsabilités

• Uses approved assessments to identify client needs and develops initial and ongoing care plans.
• Updates plans at required intervals based on client condition.
• Manages a caseload with progress notes and documentation.
• Develops resource inventories to meet client needs identified in assessments.
• Facilitates linkage, referral, and follow-up with service providers and plans.
• Engages hard-to-reach clients to accept program services.
• Provides information and advocacy on entitlements and community services.
