Job summary

SCO Family of Services seeks a Health Home Care Manager to coordinate medical, behavioral health, substance use and social services for children, adults, and families with complex needs. The hybrid role requires travel across Suffolk and Nassau Counties and NYC boroughs; candidates must reside in New York State and have the specified qualifications.

The position reports to the Care Manager Supervisor. Responsibilities include delivering NYS Health Home Core Services, engaging clients, and

Qualifications

• Care coordination and case management experience.
• CANS training and annual certification preferred.
• Bilingual Spanish preferred.

Responsibilities

• Deliver six NYS DOH Health Home Core Services including care coordination and health promotion.
• Coordinate medical, behavioral health, substance use, and social supports.
• Engage members through outreach and follow-up.
• Maintain timely documentation in NYSDOH-approved systems.
• Collaborate with hospitals, MCOs, and community partners.
• Meet productivity, engagement and quality benchmarks.
