Qualifications

• 2+ years of experience working with frail or elderly disabled adults and caregivers.
• Valid driver’s license with an insured vehicle for travel.
• Previous experience in a medical or case management environment preferred.

Responsibilities

• Perform orientations for new patients.
• Conduct initial assessments during orientations.
• Engage in interdisciplinary meetings with caregivers.

Skills

Experience with frail elderly adults

Hands-on experience in home care
