Job summary

FreedomCare in Colorado is seeking a Care Manager to join our field-based team. You will coordinate home-based care for Medicaid patients, conduct quarterly supervisory visits, and partner with caregivers to ensure safe, compliant, and uplifting home care services.

This role requires fieldwork five days a week across the Denver area, a bachelor’s degree or equivalent experience, and strong communication, problem-solving, and HIPAA awareness in a fast-paced startup environment.

Qualifications

• Bachelor’s degree or equivalent work experience.
• 2+ years bedside or direct care experience.
• 1+ years in call center or customer service is required.

Responsibilities

• Schedule and perform quarterly supervisory visits for home care compliance.
• Support members and caregivers in continuing care and utilization of hours.
• Conduct home safety inspections and document in Salesforce.
• Communicate concerns to RN or Case Manager and maintain care plans.
