Job summary

The Child Center of NY seeks a Resource Coordinator (Care Manager) to join our NYC team. You will participate in outreach and enrollment and guide individuals, families, and caregivers to the services they need.

As part of an inter-disciplinary team, you will coordinate medical, social, and behavioral health supports, conduct home visits, and help ensure access to community resources while ensuring compliance with regulations.

Qualifications

• Bachelor’s degree in Human Services or related field.
• 2 years of experience in Human Services or related field with course work in social work/psychology.
• Ability to plan and carry out assignments independently.
• Home visits are required.

Responsibilities

• Complete intake and enrollment and monitor ongoing assessments.
• Coordinate interdisciplinary collaboration among providers, enrollee, family and caregivers.
• Provide information on services and entitlements; refer to community resources.
• Advocate for individuals/families as needed between programs.
• Conduct office/home/community visits to assess needs and service delivery.
• Perform outreach to program candidates within the community.
• Coordinate services for families with medical or mental disabilities and ensure compliance with regulations.
