- Washington Clinical Independent Social Worker Associate License.
- 3 years of social services experience in a health care related organization; include minimum 2 years in acute care/care coordination.
Responsibilities
- Coordinate care for complex designated patients from pre-admission to immediate post-discharge.
- Facilitate communication between the health care team, patient and family to minimize fragmentation.
- Provide psychosocial services and support to patients and families as part of the care coordination team.
