Qualifications

• Master's degree in social work from an accredited CSWE program.
• Upon hire: Washington Clinical Independent Social Worker Associate License.
• 1 year of SW experience in any healthcare setting.
• 1 year of experience in acute care setting or successful completion of TIPs program or Case Management Orientation Program.

Responsibilities

• Assess and plan care for at-risk patients in an acute care environment.
• Coordinate care and discharge planning to ensure continuity of care.
• Document interventions and maintain regulatory compliance.
• Advocate for patients and identify resources to address barriers to care access.

Education

Master's degree in social work
