Job summary

Centene is seeking a Senior Care Manager to develop, assess, plan, implement, and coordinate complex care management activities focused on behavioral health needs. This fully remote role combines clinical judgment with high-acuity care delivery.

You will manage care plans via phone and in-home visits, coordinate with providers, monitor outcomes, and document all activities. Requires a Master's in Behavioral Health or Social Work, 4–6 years of behavioral health experience, and an appropriate

• Coordinate care between members, families, and the care provider team.
• Monitor outcomes and identify barriers to care coordination.
• Review referrals and intake assessments to update plans.

About This Role

The Senior Care Manager develops, assesses, plans, implements, and coordinates complex care management activities focused on behavioral health needs to provide quality, cost-effective healthcare outcomes. This fully remote position is ideal for individuals with strong clinical judgment and experience in behavioral health.

Highlights

• Pay: $75,300 - $135,400/year
• Location: Remote, NY
• Schedule: Full Time, Day shift

What You'll Do

• Develop and continuously assess ongoing care plans/service plans for members with high acuity via phone or in-home visits.
• Perform telephonic, digital, home, and/or other site visits outreach to assess member needs.
• Coordinate and manage care between the member and/or family and the care provider team.
• Maintain a caseload with a higher volume of complex/specialty cases.
• Monitor member status and outcomes for changes in treatment side effects, complications, and clinical symptoms.
• Identify problems/barriers for care coordination and appropriate care management interventions.
• Review member data to identify health risks and/or care gaps.
• Review referrals information and intake assessments to develop appropriate care plans.
• Collaborate with healthcare providers and partners to facilitate care coordination.
• Collect, document, and maintain all member information and care management activities.

Requirements

Education:

• Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing required.

Experience:

• 4-6 years of related experience required.
• Behavioral health experience required; experience working with children and families preferred.

Licenses & Certifications:

• One of the following licenses required: LCSW, LMSW, LMFT, LMHC, LPC, or RN based on state contract requirements.
