Job summary

Qualifications

• Bachelor’s degree in Human Services or an accredited field.
• Credentials: RN, LBSW, LMSW, LPC or similar licensure as listed in the qualifications.
• QMHP/QIDP/CMHP credentials required or preferred depending on population served.

Responsibilities

• Gather and review medical, psychological, and social history to assess needs toward least restrictive environment and services.
• Initiate and coordinate professional assessments and share data with agencies, courts, and caretakers.
• Prepare person-centered plans of service and periodic progress reviews reflecting client preferences.
• Develop short- and long-term goals and oversee implementation across residential, employment, school and other settings.
• Advocate for individuals and liaise with families, guardians, caretakers, and service agencies.
