Qualifications

• 2+ years of relevant care management experience.
• Bachelor’s degree in social work, sociology, psychology, gerontology, or related field.
• RN licensed in Florida or LPN with 4+ years' Florida licensure.
• EHR proficiency and basic computer skills.

Responsibilities

• Assess member needs and develop person-centered care plans.
