A First Name Basis Home Care is seeking a dedicated Care Manager to lead individualized care planning and coordinate services for clients in the community.

The role emphasizes supervision of staff, collaboration with community resources, and rigorous documentation to ensure quality care across multiple offices. A strong candidate will communicate effectively and manage priorities in a fast-paced homecare environment.

Qualifications

• 2+ years of experience in homecare preferred.
• High school diploma required; Bachelor’s degree preferred or equivalent work experience.
• Excellent written and verbal communication skills.
• Reliable vehicle for travel within area.
• Ability to work in a fast paced, multi-task environment with competing priorities

Position Summary
