A First Name Basis (AFNB) is seeking a dedicated Care Manager to join our in-home care team. You will develop individualized care plans, supervise staff, and coordinate services to support seniors and individuals with disabilities across multiple offices.

Qualifications

• 2+ years of experience in homecare preferred.
• High school diploma required; Bachelor’s degree preferred or equivalent work experience.

Position Summary:
