Job summary

Qualifications

• Associate degree in a related field; one year of direct service to SED children or linkage to services for mental health.
• CANS certification to be completed and renewed yearly.
• Strong ability to read, interpret, and document client progress notes and reports.

Responsibilities

• Perform CANS Assessment and Health Home Screening on new referrals to determine Health Home admission.
• Maintain client consents for services and coordinate with Health Home on client needs.
• Develop client Plans of Care with DOH required elements by due dates.
• Attend mandated Health Home and DOH trainings; document within 5 days of client contact.
• Travel to Rockland County to provide care management as required.

Job Summary

The Care Manager is responsible for the Care Coordination of Targeted Case Management Cases. This includes assessment of new referrals, outreach to gain referrals and coordinating medical and mental health services with assigned Health Home.

Duties & Responsibilities

• Perform CANS Assessment and Health Home Appropriateness Screening Assessment on all new referrals, determining qualifications for Health Home admission
• Maintain all necessary Client consents for service
• Conduct outreach in designated venues to generate program referrals
• Utilization of web based: Health Commerce System MAPP, UAS CANS and Health Home, GSI software
• Coordinate with Health Home on Consumer service needs
• Demonstrate an understanding and put into practice the 5 elements of client Health Home service delivery
• Conduct mandated monthly client contacts
• Attend necessary appointments with clients
• Develop client Plan of Care by the required due dates to include the DOH required 10 Care Plan elements
• Follow all Health Home guidelines, policies and procedures

Attend all mandated and necessary Health Home and DOH training

• Adhere to Agency contemporaneous documentation timeframe of 5 days from point of client contact
• Adhere to Agency Humans Resources Policies and Procedures
• Maintain required agency yearly training hours
• Work flexible hours when required
• Attend weekly Supervision
• Perform other related tasks as indicated by Supervisor and Executive Vice President
• Travel to Rockland County to provide care management is required
• Ability to interact with and complete assignments in a manner sensitive to the service population’s cultural and socioeconomic characteristics.

Education & Experience

Associate Degree in one of the following Fields: Audiology, Child and Family Studies, Communication Disorders, Community Mental Health, Counseling, Education, Nursing, Occupational Therapy, Physical Therapy, Psychology, Recreation, Recreational Therapy, Rehabilitation, Social Work, Sociology, Speech and Language Pathology, Human Services, Human Development, Criminal Justice or other related degrees. One (1) year experience providing direct services to Severely Emotionally Disturbed (SED) children and adolescents, or providing linkage to services for children with one mental health diagnoses.
