Job summary

Argus Community, Inc. is seeking a Mental Health Care Manager (HH+ SMI) in the Bronx to coordinate care for up to 20 clients under NYSDOH guidelines. You will assess needs, develop client-centered care plans, and collaborate with providers and MCOs.

Responsibilities include home visits, regular contact with clients, case conferences, crisis intervention, and accurate documentation to support Health Home functions. Requires strong communication and organizational skills.

Qualifications

• Master’s degree in a qualifying field with relevant experience.
• Bachelor’s degree in a qualifying field plus 2+ years of experience or CASAC with 2+ years of experience.
• Bachelor’s degree or higher with 3+ years of Experience or 2+ years as Health Home care manager serving SMI population.

Responsibilities

• Complete initial assessment and develop a client-focused care plan.
• Advocacy and knowledge of available services for participants.
• Conduct home visits (minimum 2 per month per client).
• Maintain monthly per member per month billing and document diligently.
• Coordinate with MCOs to ensure HCBS services are received.
