- In field about 50-60% of the time – individual can determine the days in field and manage their own schedule. The rest of the time will be documentation from home

- Training Monday-Friday 8a-5p EST – 4 weeks training – classroom & 1-on-1 – virtual – CAMERAS ON – NO TIME OFF during training

Responsibilities:
- Develop, assess, and facilitate complex care management activities for primarily physical needs members to provide high quality, cost-effective healthcare outcomes.
- Create personalized care plans and provide education for members and their families.
- Log on to the laptop by 8 am, review emails, tasks, and voicemails, and respond to any urgent needs.
- Review calendar for possible meetings and/or scheduled visits.
- Arrange 5-7 visits weekly and ensure documentation is completed within 24 hours.
- Meet required turnaround times for processes and complete trainings timely.
Requirements:
- Required: Bachelor’s degree in counseling or social work
- Minimum 2 years of related experience in nursing, case management, home health, or behavioral health
- Licensure: LPC, LPCC, LSW
- Reliable vehicle for weekly travel
Preferred Skills:
- Related experience with Behavioral Health
- Computer literate (knowledge of Microsoft Excel, Word, Teams, Outlook, OneNote, OneDrive, PowerPoint, Explorer, Chrome)
- Critical thinker
- Works well independently and possesses troubleshooting skills
- Strong communication skills, personable, and able to work well on a team
