Job summary

Spectraforce Technologies seeks a Care Manager (RN) to support North East Ohio members through in-home care management. This hybrid role combines remote work with required in-person home visits across NE Ohio counties.

Qualified candidates hold an Ohio RN license and have 2–4 years of nursing, case management, or home health experience. A reliable vehicle for weekly travel is required, with 5–7 visits weekly and fast documentation turnaround.

Position Title: Care Manager (RN)

Work Location: Remote - OH and In Person. Must reside in North East Ohio - preferably close to Trumbull, Stark, Seneca, Portage, Mahoning counties.

Assignment Duration: 6 months (temp to hire)

Work Arrangement: Hybrid (remote with required in-person home visits)

Position Summary: Develops, assesses, and facilitates complex care management activities for primarily physical needs members to provide high quality, cost-effective healthcare outcomes including personalized care plans and education for members and their families.

Everyone works together to ensure our members get the care and services they need to remain as independent as possible.

Background & Context: Role supports The Organization's members in North East Ohio through in-home care management.

Key Responsibilities

• Develops, assesses, and facilitates complex care management activities for primarily physical needs members
• Provides high quality, cost-effective healthcare outcomes including personalized care plans and education for members and their families
• Performs 5-7 in-home visits weekly
• Ensures documentation is completed within 24 hours
• Meets required turn-round times for processes
• Completes required trainings timely

Qualification & Experience

• Degree from an Accredited School of Nursing and 2-4 years of related experience
• Preferred: 2-4 years of related experience in case management or home health experience
• Required: Ohio RN Licensure
• Years of experience required: 2-4 years of experience in nursing, case management, home health
• Reliable vehicle for weekly travel

Working Conditions & Physical Demands (If Applicable): In-home visits required (5-7 weekly) with reliable vehicle for travel in North East Ohio counties.

Candidate Requirements

Education/Certification: Degree from an Accredited School of Nursing and 2 - 4 years of related experience

Disqualifiers: Inability to work independently, manage change well, inability to utilize technology and Microsoft Office

Additional qualities to look for: Someone who works well independently, personable, strong communication skills. Someone who can think critically, be flexible, open to change, and can also work well on a team.

During 5 week training they will not be able to miss any days of training. They must be able to attend ALL training classes and modules.
