The 80th Street Residence in Florida seeks a Care Manager to provide quality professional care management for clients and their responsible parties. You will help navigate aging challenges and disabilities to maximize quality of life, coordinating care, supervising aides, and interfacing with medical staff.

Role emphasizes advocacy, information sharing, referrals, and compassionate, individualized service delivering support across families and healthcare teams.
