UNC Lenoir Health Care in North Carolina is seeking a Case Management Assistant to support the Care Management team under the Director. The role focuses on organizing workflow, communications, and linking patients with community resources, ensuring efficient operations.

This per diem, onsite position requires strong organizational skills, medical terminology knowledge, and ability to work under pressure with hospital staff and payers. 4 hours per week expected.

Qualifications

• High school diploma or equivalent required.
• 2-3 years of secretarial or administrative assistant experience preferred.
• Advanced education beyond high school in business or healthcare is a plus.

Responsabilités

• Support case management workflow, communications, and information tracking.
• Coordinate with hospital staff, patients, payers, and community resources to project a positive image.
• Assist in discharge planning and ensure accurate documentation and data entry.
