Job summary

CommonSpirit Health in Lincoln, Nebraska seeks a Director of Care Management to provide strategic leadership and comprehensive operational oversight for all care management programs across CHI Health facilities. You will lead multidisciplinary teams to optimize patient flow, resource use, and discharge planning while ensuring regulatory compliance and high-quality, cost-effective care.

Qualifications

• Bachelor's degree required.
• One of RN/ CMSW/ LICSW/ LISW/ LSW/ MSW/ PLMSW/ SWORKER licensed in NE.
• Mandatory Reporter (Adult & Child) certification within six (6) months if working in or traveling to Iowa, or a Behavioral Health/Psych unit.
• Excellent customer service and presentation skills.
• Strong interpersonal and written communication skills.
• Demonstrated analytical and problem-solving ability.
• Demonstrated ability to manage multiple tasks/projects effectively.
• Ability to work independently with a high degree of detail orientation.
• Capable of planning and organizing projects with short notice.

Responsibilities

• Directs and manages all Care Management functions—planning, staffing, performance improvement (e.g., LOS reduction), and regulatory/government reporting for the medical center and CHI Health.
• Ensures Care Management practices align with medical center, health plan, Federal/State, and professional standards.
• Works with physicians, administrators, managers, and community healthcare partners to address care management issues, and represents the department in facility, division, and corporate meetings/strategic planning.
• Partners across departments and with post-acute providers to ensure smooth patient transitions through discharge and into the community, optimizing coordination across the full continuum of care.
• Actively supports the organization's Mission, Core Values and strategic plan.
