CHI Health St. Elizabeth seeks a Director of Care Management to provide strategic leadership and comprehensive operational oversight for care management programs across the medical center.

Job Summary and Responsibilities

As our Director of Care Management for both St. Elizabeth Hospital and Nebraska Heart Hospital, you will be a central figure providing strategic leadership and comprehensive operational oversight for all care management programs, ensuring integrated, high-quality, and cost-effective patient care across the continuum.

Every day, you will expertly manage multidisciplinary care coordination teams, collaboratively developing and implementing strategic plans to optimize patient flow, resource utilization, and discharge planning. You will also oversee regulatory compliance, quality metrics, and cultivate strong relationships with physicians, payers, and post-acute providers, driving innovation while maintaining high standards of patient advocacy.

To be successful in this role, you will combine robust healthcare leadership experience, a proven track record in comprehensive care management, and an exceptional ability to inspire diverse clinical teams. You will demonstrate a strategic mindset for system-level planning, strong financial acumen related to value-based care, and thrive in a dynamic, high-growth environment dedicated to transforming healthcare delivery.

  • Directs and manages all Care Management functions—planning, staffing, performance improvement (e.g., LOS reduction), and regulatory/government reporting for the medical center and CHI Health
  • Ensures Care Management practices align with medical center, health plan, Federal/State, and professional standards
  • Works with physicians, administrators, managers, and community healthcare partners to address care management issues, and represents the department in facility, division, and corporate meetings/strategic planning
  • Partners across departments and with post-acute providers to ensure smooth patient transitions through discharge and into the community, optimizing coordination across the full continuum of care
  • Actively supports the organization's Mission, Core Values and strategic plan
Job Requirements

Required

  • Bachelor's degree
  • One (1) of the following licenses/certifications:
    • Registered Nurse (RN:NE)
    • Certified Master Social Worker (CMSW:NE)
    • Licensed Independent Clinical Social Worker (LICSW:NE)
    • Licensed Independent Social Worker (LISW:NE)
    • Licensed Social Worker (LSW:NE)
    • Master Social Worker (MSW:NE)
    • Provisional Licensed Master Social Worker (PLMSW:NE)
    • or Social Worker (SWORKER:NE)
  • Mandatory Reporter (Adult & Child) certification within six (6) months, if working in or traveling to Iowa, or a Behavioral Health/Psych unit
  • Excellent customer service and presentation skills
  • Strong interpersonal and written communication skills
  • Demonstrated analytical and problem-solving ability
  • Demonstrated ability to manage multiple tasks/projects effectively
  • Ability to work independently with a high degree of detail orientation
  • Capable of planning and organizing projects with short noticeAbility to work efficiently in a fast-paced environment with changing priorities

Preferred

  • Master's degree
  • Accredited Case Manager (ACM) certification
