CHI Health St. Elizabeth seeks a Director of Care Management to provide strategic leadership and comprehensive operational oversight for care management programs across the medical center.
You will lead multidisciplinary teams, optimize patient flow and discharge planning, ensure regulatory compliance and quality metrics, and collaborate with physicians, payers, and post-acute partners to deliver high-value, patient-centered care.
As our Director of Care Management for both St. Elizabeth Hospital and Nebraska Heart Hospital, you will be a central figure providing strategic leadership and comprehensive operational oversight for all care management programs, ensuring integrated, high-quality, and cost-effective patient care across the continuum.
Every day, you will expertly manage multidisciplinary care coordination teams, collaboratively developing and implementing strategic plans to optimize patient flow, resource utilization, and discharge planning. You will also oversee regulatory compliance, quality metrics, and cultivate strong relationships with physicians, payers, and post-acute providers, driving innovation while maintaining high standards of patient advocacy.
