Job summary

Trillium Health Resources is hiring a Care Management Specialist I to join the Care Management team. This remote-friendly role supports members in the South Central region by assisting Care Managers with outreach, care planning, and connecting members to medical and community resources.

The position offers typical hours 8:30 am–5:00 pm with travel within the catchment as needed, and benefits including health insurance, PTO, and 401k match. This role emphasizes service and community impact.

Qualifications

• High school diploma or GED and one year of lived experience with an intellectual/developmental disability (I/DD) or a traumatic brain injury (TBI) and knowledge of the NC Medicaid delivery system OR one year of lived experience with a behavioral health condition with certification as a Peer Support Specialist OR one year of direct experience caring for a person with I/DD, TBI, or behavioral health condition navigating Medicaid.
• Must have a valid driver’s license and reside within Trillium’s South Central Region (counties listed).
• Must be legally authorized to work in the United States without sponsorship now or in the future.

Responsibilities

• Provide outreach, follow-up, and engagement with members.
• Arrange/schedule member services, appointments, and non-emergency transportation.
• Collaborate with Care Managers to communicate health updates and changes.
• Attend educational case conferences and ensure proper handling of health records.
