Job summary

The Ohio State University Wexner Medical Center is seeking a Care Management Manager to oversee daily operations of care management and social services from preadmission to post-discharge. You will direct Care Managers, Utilization Care Managers, and Social Workers across hospital units, coordinating high quality discharge planning and patient care optimization.

Successful candidates will have a bachelor’s degree (master’s preferred), RN or LMSW licensure, and at least 2 years of clinical

Qualifications

• Bachelor’s degree required; Master’s degree preferred.
• RN or LMSW required.
• Minimum 2 years clinical experience in acute care, home care, hospice, community agency, or payer relations.
• Minimum 3 years of leadership or management experience.
• Strong communication and interpersonal skills; ability to solve complex problems.
• Must understand the health care continuum and work with various computer applications.
• Case Management Certification preferred.

Responsibilities

• Oversee daily operations of care management and social services teams from preadmission to post-discharge.
• Direct Care Managers, Utilization Care Managers, and Social Workers across multiple hospital units and departments.
• Coordinate high quality, safe, timely, efficient discharge planning.
• Collaborate with the Director to implement innovative strategies to improve patient care, throughput, outcomes, and cost efficiency.
