Job summary

Responsibilities

• Facilitates patients through the health care system from pre-admission to post-discharge coordination of care and resources.
• Monitors and coordinates the patient plan of care to improve timeliness and care quality.
• Identifies and removes barriers to progression of the plan of care for each patient.
• Collaborates with physicians, nurses and other health care workers to manage transitions of care.
• Serves as the central information source for third-party payers, communicating the plan of care and medical necessity and gaining approval for post-discharge care.
