Job summary

The Ohio State University Wexner Medical Center is seeking a Care Management Manager to oversee daily operations of care management and social services from preadmission to post-discharge. You will direct Care Managers, Utilization Care Managers, and Social Workers across hospital units, coordinating high quality discharge planning and patient care optimization.

Job description

Scope of Position/Position Summary:

The Care Management Manager is responsible for overseeing daily operations and promoting teams in care management and social services from preadmission to post discharge by directing Care Managers, Utilization Care Managers, and Social Workers, within aligned teams across University, Brain & Spine, Ross, Dodd, OSU East hospitals and emergency departments. The manager is responsible for assuring teams facilitate and coordinate high quality, safe, timely, efficient and effective social services and discharge planning as applicable. The Manager works closely with the Director to assure innovative strategies are used positively to impact patient care, throughput, achieve quality patient outcomes; improve service performance and reimbursement; and length of stay and costs.

Minimum Qualifications:

Bachelor’s degree required, Masters preferred. Registered Nurse or Licensed Masters Social Work required. Minimum 2 years clinical experience applicable to the patient population in acute care setting, home care, hospice, community agency, and/or payer relations required. Minimum 3 years of leadership or management experience. This role must understand the health care continuum, have the ability to solve complex problems, and possess strong communication and interpersonal skills. Must be able to work with various computer applications. Case Management Certification preferred.
