OU Health is seeking an Adult Care Management RN for the Emergency Department. The role supports patient populations through assessment, planning, coordination, and evaluation to meet health needs and promote quality outcomes.

Positions may be filled at Levels 1–3 based on education, experience, and license. Competitive wages, sign-on bonus, and potential relocation assistance available for those outside 100 miles.

Qualifications

• RN license issued by the Oklahoma State Board of Nursing or a current multistate compact (eNLC).
• Basic Life Support from the American Heart Association required.
• Level 1 requires ADN or higher; Level 2/3 require BSN with related CM experience.

Responsabilités

• Conduct comprehensive assessments of patients’ health status and care needs.
• Develop individualized care plans with the healthcare team and families.
• Educate patients/families on conditions, plans, medications, and self-care.
• Coordinate communication between patients, families, providers, and resources.
• Evaluate and adjust care plans for quality and cost-effectiveness.
• Monitor utilization and optimize care planning; advocate for resources.
• Ensure timely authorization and coverage with insurers and social workers.
• Participate in quality improvement and patient safety initiatives.
