Job summary

The Neuron Clinic is a busy outpatient neurology practice with locations in Chula Vista, El Cajon, Murrieta, Corona, Palm Springs and San Marcos, CA. We are seeking an infusion nurse with leadership abilities, ideally with experience in neurology or oncology, to join our care management team.

This is a full-time remote-capable role within California, focusing on delivering care management to patients enrolled in CCM and PCM programs while collaborating with referrals and authorizations teams.

Qualifications

• A minimum of one year of experience in care management or case management is preferred.
• A minimum of one year of experience in an outpatient setting is preferred.
• Bilingual English/Spanish preferred.
• Current and unrestricted California Licensed Vocational Nurse (LVN) license preferred.

Responsibilities

• Maintain a working expert knowledge of the care management scope of practice, and perform all tasks in accordance with it
• Maintain sufficient knowledge of The Neuron Clinic’s available care management programs to perform the tasks required of each program
• Work closely with our internal Referrals and Prior Authorizations department to ensure that patient orders can be successfully completed
• Manage an active panel of complex neurological patients enrolled in care management, including but not limited to patients with neuroimmunological disorders, patients with Alzheimer's and dementia, patients with movement disorders, and patients receiving infusion therapy at The Neuron Clinic

Job Summary

The Care Management works under the supervision of a licensed Physician to deliver care management to patients enrolled in both The Neuron Clinic's Chronic Care Management (CCM) and Principal Care Management (PCM) care programs. This role is primarily responsible for operating our care management programs, including directly managing a panel of enrolled patients and engaging with the patients monthly according to their clinician’s plan of care.

Work Location
