The Neuron Clinic is a busy outpatient neurology practice with multiple CA locations, seeking an infusion nurse with leadership abilities. Flexible remote work within California is available.

The role centers on operating our CCM and PCM programs, managing an enrolled patient panel, and engaging monthly per the clinician’s plan of care. Qualified candidates have 1+ year in care management and outpatient settings, Bilingual English/Spanish preferred, and CA LVN license preferred.

Qualifications

• Minimum one year of care management or case management experience.
• Minimum one year of outpatient setting experience preferred.
• Bilingual English/Spanish preferred.
• Current and unrestricted California LVN license preferred.

Responsibilities

• Maintain expert knowledge of care management scope of practice and perform tasks accordingly.
• Maintain knowledge of available care management programs to execute required tasks.
• Collaborate with Referrals and Prior Authorizations to ensure patient orders are completed.
• Manage a panel of complex neurological patients enrolled in care management and engage monthly per clinician's plan.

Job Summary

The Care Management works under the supervision of a licensed Physician to deliver care management to patients enrolled in both The Neuron Clinic's Chronic Care Management (CCM) and Principal Care Management (PCM) care programs. This role is primarily responsible for operating our care management programs, including directly managing a panel of enrolled patients and engaging with the patients monthly according to their clinician’s plan of care.

Responsibilities

• Maintain a working expert knowledge of the care management scope of practice, and perform all tasks in accordance with it
• Maintain sufficient knowledge of The Neuron Clinic’s available care management programs to perform the tasks required of each program
• Work closely with our internal Referrals and Prior Authorizations department to ensure that patient orders can be successfully completed
• Manage an active panel of complex neurological patients enrolled in care management, including but not limited to patients with neuroimmunological disorders, patients with Alzheimer's and dementia, patients with movement disorders, and patients receiving infusion therapy at The Neuron Clinic

Qualifications

• A minimum of one year of experience in care management or case management is preferred
• A minimum of one year of experience in an outpatient setting is preferred
• Bilingual English/Spanish preferred
• Current and unrestricted California Licensed Vocational Nurse (LVN) license preferred

Work Location

This is a Full-time position and is eligible to be performed remotely from within the State of California.
