Physicians Regional Healthcare System-Collier in Naples, FL is seeking a Director, Care Management to lead the Care Management and Resource Management programs. You will ensure regulatory compliance, develop efficient discharge planning processes, and provide strong leadership to the care team.

The role requires collaboration with staff, physicians, and community resources to deliver patient-centered care and achieve optimal outcomes. Evening/weekend on-call duties may apply.

Qualifications

• RN and state licensure required or eligible to practice in Florida.
• 3–5 years in Care Management with leadership experience.
• Strong knowledge of regulatory standards and discharge planning.

Responsibilities

• Provide leadership and direction to the Care Management team.
• Ensure compliance with regulatory and accreditation standards.
• Oversee chart reviews for severity and service intensity.
• Coordinate discharge planning with social services and physicians.
• Monitor outcomes and drive process improvements across the program.
• Collaborate with hospital leadership and community resources.
