Job summary

Physicians Regional Healthcare System in Naples, FL seeks a Director of Care Management to lead planning, coordination, and oversight of care management services, including utilization review, discharge planning, and transitions of care. You will supervise staff, collaborate with physicians and payors, and drive outcomes aligned with patient needs and organizational goals.

The role requires strategic leadership, regulatory compliance, and strong communication to ensure efficient, cost-effective

Qualifications

• RN - valid state licensure (or equivalent) required.
• MSW/LMSW/LCSW licensure required or in process.

Responsibilities

• Oversee daily operations of the Care Management department in coordination with the Director.
• Oversee utilization review and discharge planning processes for appropriate admission and discharge.
• Direct and prioritize workloads based on patient length of stay, DRGs, charges, and reimbursement.
• Evaluate patient status via chart review and team interviews to support care planning.
• Collaborate with physicians, nurses, payors to streamline care and timelines.
• Monitor observation stays and ensure regulatory compliance and timely decisions.
• Prepare and deliver reports on utilization and performance to leadership.
• Perform other duties and maintain reliable attendance and adherence to policies.
