Qualifications

• Bachelor's Degree in Nursing required;
• Master's Degree preferred;
• 3-5 years experience in Care Management required;
• 3-5 years of leadership experience preferred.

Responsibilities

• Oversees the implementation and maintenance of a compliant Care Management program meeting regulatory standards.
• Provides leadership to staff and ensures accountability within scope of practice.
• Conducts chart reviews to evaluate severity of illness and intensity of services.
• Coordinates discharge planning and social services to ensure compliance with laws and standards.
• Acts as liaison to community agencies for post-hospital placement and resources.
• Participates in Patient Care Conferences and LOS meetings.
