Job summary

Hackensack Meridian Health is seeking a Care Coordinator, Care Management in New Jersey to coordinate and facilitate clinical progression of treatment and discharge plans. You will partner with patients, families and a multidisciplinary team to ensure appropriate next levels of care and timely transitions.

The role requires a BSN (or progress) or MSW, strong decision-making and communication abilities, plus proficiency with Epic and other clinical tools.

Qualifications

• BSN or BSN in progress and/or willing to acquire within 3 years
• Masters Degree in Social Work is acceptable/preferred

Responsibilities

• Assess patients and develop individualized care plans with the physician and health care team
• Coordinate and communicate across multidisciplinary teams to ensure smooth discharge planning
• Maintain up-to-date information on community resources and referrals
• Support timely transitions to the next level of care and document care coordination activities
• Educate patients and families on discharge options and available health care benefits
