Job summary

AdventHealth Kissimmee is seeking a patient care coordinator to promote excellent customer service through interactions with patients, physicians, care managers and the healthcare team.

Under the direction of a RN or Care Manager, you will coordinate patient care and post-acute services, arrange transportation, obtain insurance authorizations, and document regulatory letters to patients or representatives as required by regulations.

Qualifications

• High school diploma or equivalent required.
• 1+ office experience preferred, preferably in a medical setting.
• Previous data entry experience preferred.

Responsibilities

• Promotes excellent customer services through interactions with patients, physicians, care managers, and the healthcare team.
• Coordinates patient care and services arrangements under the direction of a RN or SW Care Manager and in collaboration with the patient/family.
• Obtains insurance authorization for patient post-acute care needs as directed by the Care Manager.
• Arranges patient transportation and follow up appointments.
• Documents delivery of regulatory letters to patients or their representatives in accordance with Federal and State regulations.

Job Description:
