Job summary

Centene Corporation seeks a field-based care coordinator to close care gaps by partnering with members and providers in Little Rock, Arkansas. You will educate practices on HEDIS measures, coordinate preventive services, arrange transportation, and support wraparound services while safeguarding member privacy.

This role requires strong collaboration with provider offices, data review for quality improvement, and effective communication to resolve issues.

Qualifications

• Bachelor's degree in Healthcare, Public Health, Nursing, Psychology, Social Work, Health Administration, or related health field; total 4 years of experience required.
• 2+ years direct patient care/health coaching in a managed care environment preferred.

Responsibilities

• Acts as liaison between member/family, physician and facilities/agencies.
• Educates providers on HEDIS measures and coding per NCQA requirements.
• Schedules doctor appointments for members with care gaps.
• Arranges transportation and wraparound services for members.
• Documents actions and coordinates with customer service to resolve issues.
• Travels as needed to provider offices and member sites.
