Job summary

Nurse Next Door is seeking a Care Designer to drive client experience and caregiver management. You will lead day-to-day operations to grow the business through exceptional service and quality care.

Responsibilities include client consultations, care plan development, scheduling coordination, and leading a caregiving team to deliver Happier Aging. On-call and flexible full-time availability required.

Qualifications

• Nursing background is an asset.
• CPR and First Aid Certification.
• Car and valid driver's license.
• Background check with Vulnerable Sector Search updated every 5 years for continued employment.
• Negative TB test updated every 5 years for continued employment.

Responsibilities

• Conduct Caring Consults and assess clients with respect to their physical, emotional, and environmental needs.
• Advise clients on appropriate care and achieve a 90% conversion goal.
• Drive business interest by engaging with community leaders and professional organizations.
• Develop and promote high-quality, innovative customer service.
• Act as an advisor during Caring Consults to ensure clients have all information they need.
• Attend First Visits with caregivers to ensure care plans are executed as per Service Agreement.
• Set and role-model expectations for caregivers.
• Conduct supervisory visits to ensure caregiver fit and quality of care while achieving a high Net Promoter Score.
• Develop individualized care plans and update them as required.
• Educate clients on self-care and evaluate responses to care.
• Monitor risk issues and adapt care as needed.
• Coordinate care and evaluate client outcomes based on goals.
• Lead caregiving team through goal-setting and coaching.
