Care Coordinators are part of the WISe (Wraparound with Intensive Services) program, which serves youth who have mental health diagnoses and are experiencing severe behavioral disruptions that impact their functioning in family, school, and social environments. WISe is an evidence-based model with a collaborative, cross-system approach to creative problem-solving. Care Coordinators work with youth and their families to develop and implement individualized, cross-system care plans that work toward the family’s long-term goals. Services are provided where and when convenient for the family, sometimes out in the community and outside of regular business hours. Care Coordinators must be skilled in managing team conflict, engaging a variety of personalities, facilitating meetings, and communicating with various audiences.
This position will have a varying work schedule of mostly Monday-Friday 8AM - 5PM day shift, but also requires work outside of standard business hours to meet the needs of client families served. Although based in our Ellensburg outpatient center, Care Coordinators may meet with individuals and families out in the community at a location that's most convenient for the family, so regular travel throughout Kittitas County and Yakima County is expected.
