Job summary

Segue Health is seeking a compassionate Care Coordinator to support patients in the Longview, TX area, coordinating post-discharge transitional care management. The role combines clinical knowledge with strong organizational and communication skills to ensure smooth transitions from hospital to home.

Ideal candidates have experience in social work, nursing, or home health, with proficiency in Microsoft Excel and the ability to learn EMR/EHR software. Transportation to local hospitals is required.

Qualifications

• Must have experience in the medical field.
• Proficient in Microsoft Excel; ability to learn EMR/EHR software.
• Should have transportation to and from local hospital.
• Experience in social work, nursing, and/or home health preferred.
• Strong attention to detail and organizational skills.

Responsibilities

• Coordinate with referring facilities to generate list of patients eligible for TCM services.
• Visit referrals in the facility to explain the TCM program and assess post-discharge needs.
• Daily logging of referrals and discharges.
• Create charts in EMR and upload face sheet, H&P, and DC Summary.
• Post-discharge calls to patients within 24 hours.
• Schedule patients for in-home visits, preferably before discharge.

Segue Health, a faith based medical practice, is seeking a compassionate Care Coordinator to join our team serving patients in the Longview, TX area coordinating post discharge transitional care management. This position requires a mix of clinical knowledge, excellent organizational skills, and excellent communication skills.

Qualifications

• Must have experience in the medical field
• Must have a knowledge of Microsoft Excel and the ability to learn EMR/EHR software
• Should have transportation to and from local hospital
• Must have a heart to serve others
• Daily tasks include:
  • Working with referring facility to generate list of patients eligible for TCM services
  • Visiting referrals while in the facility to explain the TCM program and determine if there are any anticipated needs post discharge
  • Daily logging of referrals and discharges
  • Creating charts in EMR and uploading face sheet, H&P, and DC Summary
  • Post discharge calls to patients within 24 hours
  • Scheduling of patients preferably prior to discharge for in home visits
• Our most successful candidates have experience in social work, nursing, and/or home health.
• Attention to detail and organizational skills are MOST important.

Job Type: Full-time

Salary: $45,000.00 - $55,000.00 per year
