Behavioral Health Network, Inc (BHN) in Western Massachusetts is seeking a Care Coordinator I or II to apply clinical expertise to assessments, care planning, and guiding a multidisciplinary team in providing intensive supports to youth and families.

You’ll help families access services, build stability, and achieve goals, with emphasis on coordinating medically necessary supports and transitions when goals are met.

Formación

• Bachelor’s or Master’s degree in a Human Services field, license-eligible.
• CANS-MA certification required (or ability to obtain on hire).
• 1–3 years of behavioral health experience with youth and families.
• Knowledge of SED and evidence-based treatment approaches.

Responsabilidades

• Conduct initial and updated assessments, including CANS, CRAFFT, diagnostic assessments, and SED determinations.
• Convene and facilitate Care Planning Team (CPT) meetings to develop ICPs.
• Work with youth and families to identify strengths, needs, and strategies.
• Ensure Risk Management/Safety Plans are maintained.
• Coordinate with state agencies, schools, primary care, and community providers.
• Coordinate services and ensure access to medically necessary supports.
• Prepare youth and families for transitions when goals are met.
• Monitor caseload, documentation, and service delivery to program standards.
