Job summary

Central Minnesota Mental Health Center in Saint Cloud is hiring a Care Coordinator to join our Access team. This full-time role coordinates client care, connects individuals with services, and supports treatment planning across programs.

Pay Range : starting at $21.41 hourly



Schedule: Full Time between Clinic Hours Monday – Thursday: 7:30 AM – 6:00 PM Friday: 7:30 AM – 12:30 PM



HOW YOU’LL MAKE AN IMPACT


  • The Care Coordinator reports to the Director of Access

  • Provide culturally appropriate care coordination for clients as needed, coordinating with referral source to assess best services for client.

  • Assess, refer, coordinate, monitor services and communicate regularly with all providers involved to ensure appropriate care, reduce duplication of services, and promote client autonomy.

  • Maintain current knowledge of the mental health service system with the ability to locate information and resources for a variety of client needs, including medical, housing, employment, social services, and community programs.

  • Assist clients with insurance

  • Complete all required documentation by specified deadline

  • Provide support and coordination for clients who are on the Adult Clinical Care Depression Pathway.

  • Assist with treatment planning process by coordinating treatment planning meetings and assisting in goal achievement.

  • Assist clients in addressing barriers to accessing services, including transportation, insurance, finances, and legal issues.

  • Attend weekly team meetings in assigned location to collaborate with providers.

  • Meet with Director of Access monthly

  • Assist support staff and clinicians in identifying appropriate referrals and resources for clients or potential clients

  • Responsible for care coordination, whether it involves coordination within CMMHC’s programs or with another entity identified in the Certified Community Behavioral Health Clinic (CCBHC) statutory language related to care coordination.

  • Represent the agency in outreach activities by build strong ties with local groups, schools, and businesses.



WHAT YOU’LL NEED

WHAT WILL SET YOU APART


  • 2 or more years of Administrative and Customer Service experience in a client-facing role preferred

  • Experience with multiple phone lines preferred

  • Associate’s degree in medical office administration preferred

  • Outgoing personality and skilled at building relationships.

  • Flexibility and tolerance for consistent change throughout your work week.

  • Ability to work independently.

  • Knowledge of community and area resources.
