Qualifications

• Bachelor's degree in social work required.
• Social Worker Certification required.
• 2+ years of experience in a healthcare setting.
• 2+ years of knowledge in healthcare terminology.
• Ability to read, write, speak, and provide emotional support.
• Onsite work in Waukesha, WI.
• Per diem (as needed) shift schedules during the weekend, once per month.

Responsibilities

• Coordinate patient care processes to achieve quality outcomes.
• Provide crisis intervention and patient/family support and resolution.
• Assess post-discharge needs and connect to resources.
• Develop care plans with ED and community resources.
• Educate patients and families on options within the care level.
• Report abuse and neglect as mandated by law.
• Coordinate with care team to ensure continuity of care.
• Participate in quality improvement projects.
