Qualifications

• Onsite at hospital location in Waukesha WI regional area.

Responsibilities

• Coordinate patient care with an interdisciplinary team to ensure quality outcomes.
• Provide crisis intervention and counseling to patients and families.
• Assist with discharge planning and post-hospital services.
• Identify community resources and financial assistance options.
• Educate patients and families on care options and next steps.
• Document interventions and coordinate with care team.
• Report abuse to required community agencies.
• Participate in quality improvement projects.
• Coordinate care for all age groups from newborns to elderly.
